Oswego County Launches Medically Tailored Nutrition Program for Medicaid
Eligible Medicaid members in Oswego County now have access to specialized nutrition support through a new partnership between Umoja Food For Health and Oswego County Opportunities. The initiative, funded by New York State’s 1115 Medicaid Waiver, provides tailored meals and grocery boxes to residents managing chronic or high-risk health conditions.

Chris Woods, president of Umoja Food For Health, noted that access to appropriate food serves as a critical component in managing chronic illness and improving daily patient outcomes.
