Harris County Launches Unified Health Network for Vulnerable Residents

Too many residents in Harris County currently fall through the cracks of fragmented support systems, prompting a coalition of six major health organizations to launch the Harris Collaborative. By integrating data and services, the initiative aims to bridge critical gaps in care for those with the most complex medical needs.

The partnership includes the Houston Health Department, Community Health Choice, Harris County Public Health, The Harris Center for Mental Health and IDD, Harris Health, and Healthconnect Texas. Launched on October 6 at the Healthconnect Texas headquarters, the initiative leverages a secure information exchange to provide providers with a shared, comprehensive view of patient histories. This technological backbone allows the organizations to coordinate interventions for residents who previously lacked a unified support network.

The Collaborative focuses on three high-risk demographics: individuals managing chronic illnesses like heart disease, patients with co-occurring mental and physical health conditions, and those transitioning from the Harris County Jail. By aligning resources, the group intends to intervene before patients reach a crisis point. Junaid Husain, CEO of Healthconnect Texas, noted that the initiative seeks to fundamentally shift outcomes by ensuring the right information reaches the right providers at the right time. While the pilot program is rooted in Harris County, its organizers intend to develop a replicable model for whole-person care that could eventually be adopted by communities across Texas and the nation.
